Transfer Student Health Requirements
Students transferring from an Illinois school must present a copy of their ninth grade school physical examination and a complete immunization record at the time of registration.
Students transferring from another state or country must present current immunization records and a physical examination at the time of registration.
Students transferring to an Illinois public school for the first time are required to have a vision examination conducted by a physician or licensed optometrist and submitted to Health Services.
Incoming 9th grade students must have a completed dental examination, taken place within 18 months prior to May 15 of the current academic school year.
Incoming 12th grade students must have proof of their Meningitis Vaccine after the age of 16.
