Community Referrals for School Physicals


The School Code of Illinois requires that each student entering 9th grade have a physical examination, on the Illinois Certificate of Child Health Examination Form, completed by a licensed physician, an advanced practice nurse or physician's assistant. The physical examination must include: height, weight, blood pressure, BMI, diabetic screening, signatures and dates in order to be considered in compliance. The Health History section must be completed, signed and dated by the parent / guardian of the child.

Student records must show proof of immunization series according to the guidelines of the Illinois Department of Public Health; Diptheria, Pertussis, Tetanus, Tdap, Polio, Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella and Meningitis.  The State of Illinois will periodically update vaccination requirements for school age students. New requirements will be communicated on the school website.

The physical examination is requested by June 1, 2024. You may drop it off in the Nurse's Office A106 before this date during regular business hours or fax it to 630-795-7192. A valid physical would be dated any day on or after August 16, 2023.

* There is no need to obtain a separate Physical Examination for participation in DGS sports for Freshman.  The Illinois Certificate of Child Health Examination Form is valid for thirteen months towards athletic participation.  However, an IHSA sports physical is not acceptable to meet this requirement. Please be sure to turn in a copy of the Illinois Certificate of Child Health Examination Form to both the Nurse's Office and the Athletic Office. 

Students will not be eligible to receive their Schedule of Classes unless a current physical has been submitted.

Freshman students will be excluded on the first day of school, and every day thereafter, until complete immunization records, physical examination and parent / guardian health history have been received and approved by Health Services.
