Planning for Vaccination Clinics

Dear District 99 Students and Families:

District 99 and other schools across DuPage County are beginning to plan Pfizer vaccination clinics for students who are currently age 16 or older. We are working on offering a clinic at one or both of our schools within the next month. However, other schools in DuPage County are also starting to have clinics come available and we are finding opportunities for our students to participate in their clinics as well. Parent permission will need to be provided through an authorization form in advance of receiving any vaccination and will be provided as part of registration for any vaccination opportunity. 

In order to plan appropriately, and to offer opportunities to students to be vaccinated as doses come available, we are asking ALL STUDENTS--even those younger than age 16--to fill out this survey about vaccination opportunities. Completing this survey does not commit anyone to participating in a vaccination opportunity, nor does it guarantee an opportunity. It is only for planning purposes. 

Please complete this survey using a student school Google account. 

Receiving a COVID vaccination is optional and available to students currently age 16 or older at this time. Over the summer the age range may expand and we will reach out to students and families when more opportunities come available. A decision to participate should be made after a discussion with the student and the family.
