Dear District 99 Students and Families:
Starting Monday, October 19, District 99 will begin following a Modified Hybrid Schedule. The schedule provides students the opportunity to attend school in person if health department guidelines allow. Please click here for detailed information about the revised first semester schedule and dates.
All students participating in the in-person learning opportunities must follow all safety procedures including: wearing a face covering or mask at all times; wearing a face shield on the bus, during passing periods, or when social distancing cannot be maintained; completing the self certification slip before attending school or participating in symptom checks on the bus or at arrival at school; and not using lockers or congregating in groups in hallways. Other safety measures may be introduced over time. Failure to participate in all safety measures will result in students transitioning to the fully remote learning model.
In order to have an accurate count for planning purposes, we are asking all students to complete this form:

Please fill out the form for each student by this Friday, September 25.
If families opt-out of In-Person Learning, they will remain opted-out through the end of first semester in December; however, families can opt to move into the fully remote learning format at any time.
Each student should complete their own form after consulting with their parent(s)/guardian(s).
