Downers Grove North has a certified school nurse, Mary Beth Tamm, who is on duty from 7:40 a.m. to 3:40 p.m. daily. The school nurse, under the School Code of Illinois, works closely with all staff, utilizing the school health program to contribute significantly to the attainment of optimum health and safety, so that the maximum educational potential of each student is reached. The Nurse's Office is located in the south-east corner of the building in room 115. Services include:
- Monitoring and assisting students with everyday health issues as well as those with more challenging physical needs
- Administering medications
- Maintaining and monitoring student immunization and health records
- Assigning elevator passes
- Assessing special education students along with PPS team
- Promoting awareness of health concerns to students and staff
- Maintaining and monitoring physician's orders regarding mental, physical, or emotional conditions that may limit student participation in a class and/or extra-curricular activity.
- Vision & Hearing Screening will be provided for children of mandated age/grade/groups each year. Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor. Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an examination has been administered within the previous 12 months and that evaluation is on file at the school. Vision screening is NOT an option. If a vision examination report is not on file at the school for your child, your child in the mandated age/grade/group will be screened. Please contact the school nurse if you have questions regarding the vision and hearing screening program.
Physical Examination/Immunization Requirements
Freshman Requirements:
The School Code of Illinois requires that each student entering 9th grade have a physical examination completed by a licensed physician, advanced practice nurse or physicians’ assistant, on file before beginning school. The “Medical History” section must be completed and signed by the parent or legal guardian of the student. Be sure all the remaining components of the required form are completed by your health care provider. The health record must include dates of all required immunizations.
The school administration strongly encourages parents to make an appointment early with your health care provider to avoid any unnecessary delays in complying with this school requirement. If your son/daughter plans to participate in any Athletic Programs, the physical examination must be completed after June 15th to ensure coverage for the full school year. (We encourage you to retain a copy of your child’s physical examination record with immunization dates for your personal records.)
It is important to note, if the required physical examination record has not been turned in to the Health Services office, your student will be unable to receive their registration packet and schedule on fee payment day. Students not complying with this state mandate for physical examination and immunizations will be excluded from attending school until the requirements are complete. (School absences due to non-compliance with School Code of Illinois requirements for physical examination and immunizations are considered unexcused.)
Transfer Student Requirements:
Students transferring from an Illinois school must bring a copy of their ninth grade physical and a complete and current immunization record with them to the registration appointment to be eligible for class registration. The physical examination form can be found in the “Forms” section on the right. Students transferring from out-of-the-state or country must also bring their prior school health records to the registration appointment. Beginning January 1, 2008, the Illinois School Code requires all students entering an Illinois public school for the first time must have a complete eye examination. The completed eye exam form must be returned to Health Services by October 15. The eye examination form can be found in the “Forms” section on the right.
Health concerns:
The Emergency card must be completed and signed by the parent or guardian and returned to the Health Services office. It is important to identify any health concerns, allergies or medications on this form. Throughout the school year, it is critical to immediately notify the Health Services office of any changes in emergency contact information.
If your child requires medication during the school day, a medication authorization form must be completed by the prescribing physician and the parent. Students are prohibited from carrying medications or keeping medications in their locker. Only students authorized to self-administer asthma medication or rescue epinephrine pursuant to the regulations on self-administration of asthma medications in school, are permitted to carry those medications on their person.
