Downers Grove North has a certified school nurse who is on duty from 7:40 a.m. to 3:40 p.m. daily. The school nurse, under the School Code of Illinois, works closely with all staff, utilizing the school health program to contribute significantly to the attainment of optimum health and safety, so that the maximum educational potential of each student is reached. The Nurses Office is located in the South-east corner of the building (in the new section) in room 115. If you need to reach the school nurse from outside the building please call 630.795.8480.
Commonly Requested Health Services Forms
Physical Form
The Illinois Department of Public Health has issued a two-sided form as of January 2005. The Health History section is required to be completed by parent or guardian prior to the Doctor's appointment. All completed physicals must be on file in Health Services prior to registration in August.
Public Act 93-0530 Diabetes Risk Assessment
requires that diabetes screening shall be completed as a required part of each health examination. Rules developed by the Illinois Department of Public Health require that each student, at the time of their physical be assessed for risk of having or developing Type 2 diabetes.
Illinois Certificate of Child Health Examination Code
The required Certificate of Child Health Examination form has been revised to accommodate changes in the Illinois Child Health Examination Code. Begin to use this new form immediately.
Authorization for Medication
This two-sided form has a section for asthma medication and a section for all other medication, either prescription or over-the-counter.
